Health and wellbeing coaching to better self-manage long term health conditions.
Health Connect Coaching is a new NHS programme for people living in or accessing health and social care services in Torbay and South Devon to empower and support to better mange their long-term health condition.
1:1 Coaching sessions are usually delivered online via a video call platform, from the comfort of your own home.
Your coach will be a trained volunteer who has shared lived experience of the same or similar health condition and/or symptoms and "just gets it" living with and managing the condition or symptoms.
14 sessions are delivered over a 6 month tapered but flexible apporach
We accept 3rd party and self-referrals.
Criteria for referral into Health Connect Coahcing as a peer to recieve coaching includes:
- Age 18 years and over
- Lives with one or more long-term health conditions [not primary mental health]
- Lives in or uses health and care services in Torbay and South Devon
- Wants to make a positive change to better self-manage your condition
