Health and wellbeing coaching to better self-manage long term health conditions.

1:1 Coaching sessions are usually delivered online via a video call platform, from the comfort of your own home.

Your coach will be a trained volunteer who has shared lived experience of the same or similar health condition and/or symptoms and "just gets it" living with and managing the condition or symptoms.

up to 14 sessions are delivered over a 6 month tapered but flexible apporach

We accept 3rd party and self-referrals.

Criteria for referral into Health Connect Coahcing as a peer to recieve coaching includes:

Age 18 years and over
Lives with one or more long-term health conditions [not primary mental health]
Lives in or uses health and care services in Devon
Wants to make a positive change to better self-manage your condition
Not recieveing or waiting to recieve psychological therapies e.g. counselling
