About

Social Prescribing

Tiverton Wellbeing is Tiverton Primary Care Network’s social prescribing service. Our Social Prescribers offer personalised one-to-one support and wellbeing groups to help you manage your health and wellbeing.

If you’re going through a difficult time, Tiverton Wellbeing is here to listen and provide practical, non-judgmental guidance. The team works closely with your GP, other healthcare professionals, and local community and voluntary organisations to connect you with a wide range of local services and activities that can help you feel better day-to-day.

For information about our wellbeing groups, see events.

  • A confidential, one-to-one conversation with a trained Social Prescriber who will listen to what matters to you and help identify practical steps.
  • A personalised plan that focuses on real-life goals and small, achievable changes.
  • Signposting to local services, groups and activities across the Tiverton area, from mental health and wellbeing support to education, housing and financial advice, volunteering opportunities and social groups.
  • Support to access services and to navigate referrals, forms and waitlists where needed.

What kind of things can Tiverton Wellbeing help with?

  • Employment, education, training and volunteering opportunities; links to job search support, courses and volunteering roles.
  • Housing and benefits advice, plus information about financial support that might be available to you.
  • Healthy living and weight management support and information.
