What is Hospital to Home?
Age UK Exeter have co-ordinators working in The Royal Devon University Healthcare Foundation Trust, as part of the Acute Care of the Elderly (ACE) Team currently based on Capener ward.
Patients or their families often need additional support to ensure suitable care is in place when returning home. Our co-ordinators will look to provide information & advice as to where support can be found, whatever the situation.
How can I access this support?
You will either be contacted during your stay in hospital or by a telephone call on your return home.
