Support people living with dementia and their families
At Dementia Friendly Parishes around the Yealm, we support people living with dementia and their carers across 5 rural communities around the River Yealm i.e. Wembury, Brixton, Yealmpton, Newton Ferrers and Noss Mayo and Holbeton.
We flexibly respond to the needs and interests of people who are referred to us and stay supporting them throughout their journey with dementia (or for as long as they want the support) via regular home visits, signposting etc.
We have a variety of regular dementia inclusive social groups and activities which are open to anyone in the community and are also, dementia friendly e.g. weekly gentle exercise, walking and listen and laugh (to favourite stories and poems) and monthly coffee and chat and singalong. We also work with businesses and organisations, local community groups, schools etc; supporting them to become dementia friendly e.g. via providing dementia awareness sessions, dementia friendly premises assessments etc.
